Required Screenings

Health

During this school year, the following screenings will be required or completed at school:

 Health Appraisals

  • New York State Education Law requires a health appraisal on all students entering into grades Pre-K or K, 1,3,5,7,9 & 11, and on all students newly entering the school district. New students must have a physical/health appraisal on file within 14 days of transferring in state, 30 days if  an out of state transfer. Out of state students must have a physical completed by a New York State Provider. (Click Here for the Health Exam Form)

  • Please provide a copy of your child's current physical dated within one year of the start of school to the health office. Transferring students (in state) must have a health appraisal dated within one year of date transferred.

Vision

  • Distance acuity for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7.

  • Near vision acuity and color perception screening for all newly entering students.

 Hearing

  • Hearing screening for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7.

 Scoliosis

  • Scoliosis (spinal curvature) screening for all students in Grades 5, 7. 

  

 Dental Certificates

  • A dental certificate is requested for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7.

 A letter will be sent home if there are any findings on the screening done at school that would cause concern or need medical follow-up.  Please call the school’s Health Office if you have any questions or concerns.
