    Physical Information :

New York State Education Law requires a health appraisal on all students entering into grades K, 1, 3, 5, 7, 9, & 11, and on all students newly entering the school district.  Please provide a copy of your child's current physical dated within one year of the start of school to the health office. 

Dental certificates are requested on students entering into grades K, 1,3,5, 7, & 11.

   Immunization requirements :

 New York State Public Health Law section 2164 mandates that for a child to be allowed to attend school,  proof of current and complete immunization must be provided to the school district.

   Immunization requirements for entrance into Grades 9, 10, 11, & 12 are :

Diphtheria and Tetanus toxoid containing vaccine and Pertussis vaccine
 ( DTaP / DTP / Tdap )

3 doses 

Polio vaccine
( IPV, OPV )

4 doses for grades 9, 10 , & 11( 3 doses if the 3rd dose was received at 4 years or older ),
3 doses for grade 12

Hepatitis B vaccine

 3 doses

Measles, Mumps, and Rubella vaccine
( MMR )

 2 doses

Meningococcal vaccine

1 dose for grades 9,10,& 11, and 2 doses for grade 12 (one dose if the dose was received at age 16 years or older )

 Varicella vaccine
( Chickenpox )

 Tdap
(Tetanus, Diphtheria, & Pertussis Booster)
