Except in the case of a medical contraindication determined by a licensed NYS physician, all students must have provided proof of having received all of the vaccines required by NYS health law in order to attend school. If your child has not received all of the required immunizations, THEY WILL NOT BE ALLOWED TO ATTEND SCHOOL IN PERSON OR VIRTUALLY. Listed below are the immunizations required by New York State Health Law.  Your child must have all of them to attend school in grades 9 -12. Please contact your medical provider to be sure your child has received all of their required immunizations prior to the start of the school year.

  •  3 doses of Diphtheria and Tetanus toxoid-containing Vaccine and Pertussis vaccine (DTaP /  DTP / Tdap )

  • 4 doses of Polio Vaccine (IPV or OPV) for grades 9, 10, & 11 ( or 3 doses if 3rd dose was received at 4years of age or older ), and 3 doses for grade 12

  • 3 doses of Hepatitis B Vaccine

  • 2 doses Measles, Mumps, and Rubella vaccine ( MMR )

  • 2 doses of Varicella vaccine ( Chickenpox ) 

  • 1 dose of Tdap ( Diptheria / Tetanus / Pertussis booster )

  • 1 dose of meningococcal vaccine for grades 9, 10, & 11, and 2 doses of meningococcal vaccine for grade 12 ( one dose if the dose was received at 16 years or older )

     

 Medical exemption forms are required to be submitted annually. Please contact the school nurse for further information.

Proper documentation of your child's immunizations (i.e. physician's record or prior school transcript record) must be submitted to the school nurse prior to the start of school.
