Physical exams (health appraisals) are required every year. All students must have a

current health appraisal on file from a New York State physician.

 All immunizations that are required by New York State must be given prior to entrance to school.  

Vaccines required for day care, pre-K, and school attendance

• Diphtheria and Tetanus toxoid-containing vaccine and Pertussis vaccine (DTaP or Tdap)

• Hepatitis B vaccine

• Measles, Mumps and Rubella vaccine (MMR)

• Polio vaccine

• Varicella (Chickenpox) vaccine
